We started this in September 2006 and many parents have already signed the form.  If you have not or are new to the Group, please do sign the form when you are issued with it or click on the link below and print one off.

In order to claim the money back from the taxman, we have to be able to match payments to signatories.  Therefore if more than one parent may sign cheques now or in the future, we will need both of you to sign the form.
